Immunization Requirements

In order to protect the entire Sand Tots community, all enrolled children and assisting parents must be vaccinated.

Proof of vaccinations must be submitted, approved by the Sand Tots Health Coordinator, and on file prior to a child’s first day of school.  There is no “opt-out” in lieu of vaccinations unless there is a significant medical contraindication, which must be documented by a physician.

Student Vaccine Requirements

  • 3 Polio

  • 4 DTaP (Diphtheria, Tetanus, Pertussis)

  • 1 MMR (Measles, Mumps, Rubella)

  • 1 Hib (Haemophilus influenzae type b)

  • 3 Hep B (Hepatitis B)

  • 1 Varicella/Chickenpox

Parent Requirements

  • Proof of Vaccines

    • MMR (Measles, Mumps, Rubella) or proof of immunity (based on titer test)

    • Tdap (Tetanus, Diphtheria, Pertussis)

    • Seasonal Influenza Vaccine (collected ANNUALLY between August and December)

      • Parents may exercise an opt-out in writing for this vaccine only

  • Tuberculosis Clearance

    • Evidence of a negative Tuberculin skin test (TB test) not more than one year of child’s enrollment OR

    • A Tuberculosis Risk Assessment completed by a licensed health care provider demonstrating that the individual does not present any risk factors for Tuberculosis
